Overview
Organization Description:Health Homes provide integration and coordination of primary, specialty medical care and mental health services for those who qualify for this benefit by the Washington State Health Care Authority. Health Homes are available in Snohomish, Skagit, Island, San Juan, and Whatcom counties. The goal of Health Homes is to improved health outcomes.
Health Homes interventions focus on the following six services: comprehensive care management, care coordination and health promotion, comprehensive transitional care and follow up, patient and family support, referral to community and social support services and use of information technology to link services if applicable. Care coordinators can go out to the client’s home or meet them in the community.
Intake Process:Referrals for individuals who meet the risk modeling criteria (PRISM) are generated by the Health Care Authority (HCA) and sent to local Care Coordination Organizations (CCOs). The CCOs then reach out to the individuals to offer Health Home services.
